When you contact our office, our first step is to gather all the information we can to help us determine your recommended treatment. We ask you to schedule an initial consultation with Dr. Zebovitz; and in many cases, we can reserve a date for your surgery. Then, we send you a welcome letter detailing/including the following:

• The date of your appointment, directions to our office and instructions for contacting us

• Our payment policy and insurance filing information

• The required paperwork/information you’ll need to bring to your appointment

• The appropriate forms to fill out and bring to your appointment

 

During the initial consultation, Dr. Zebovitz reviews your history, determines your needs, answers your questions and discusses your treatment options. You can generally expect the following from your appointment:

 

Wisdom Teeth Removal

• Discuss what to expect during surgery

• Review anesthetic options including:
  • Local anesthetic (with or without nitrous oxide)
  • Oral sedation
  •  IV sedation

• •Review and sign consent form authorizing the procedure

• Review pre and postoperative instructions and obtain prescriptions

• Confirm the date of surgery

• Receive written treatment proposal and review of fees

 

Implants (Single tooth replacements)

• Determine needs for site preparation

• Obtain pertinent records including dental models, x-rays or Cone Beam CT Scans

• Review anesthetic options including:
  • Local Anesthetic (with or without nitrous oxide)
  • Oral Sedation
  • IV Sedation

• Review and sign consent form authorizing the procedure

• Reserve a date for surgery, in many cases

• Review pre and postoperative instructions and obtain pre-surgical prescriptions

• Communicate treatment details to referring dentist or practitioner

• Receive written treatment proposal and review of fees

 

Surgical Consultations

• Determine needs for treatment

• Review surgical options

• Review anesthetic options including:
  • Local Anesthetic (with or without nitrous oxide)
  • Oral Sedation
  • IV Sedation

• Review and sign consent form authorizing the procedure

• Receive written treatment proposal and review of fees

 

Biopsies

• Evaluate history and findings from referring dentist or practitioner

• Determine the immediate course of action; in some cases, Dr. Zebovitz monitors the findings for a period of time before recommending a biopsy

• Schedule biopsy or perform one the same day depending upon the information presented

• Receive written treatment proposal and review of fees

 

Orthognathic Surgery Consult (Corrective surgery for conditions of the jaw or face)

• Capture and record a detailed history

• Review study models and photographs, and perform x-ray analysis

• Create computer imaging to demonstrate predicted results of proposed treatment options

• Review CAT scan and/or MRI results if applicable

• Schedule next appointment to review treatment options; discuss the insurance authorization process

 

Temporomandibular Joint Evaluations (TM Joint)

• Capture and record a detailed history

• Perform x-ray analysis

• Review CAT scan or MRI results, or consult with Dr. Zebovitz to order tests if necessary

• Coordinate treatment options if test results are available; or, schedule consultation regarding test results; finalize the treatment plan

• Schedule next appointment to review treatment options and the insurance authorization process
